MEDIA RELEASE
I hereby agree to the recording, editing, duplication, distribution, and other uses of my image and likeness, whether in whole or in part, in connection with the development, production, and promotion of SC Career Kids, and/or any other future program or production of SC Career Kids, Safety Town, South Carolina ETV, and/or South Carolina First Steps. These photos and/or recordings may appear on printed or promotional materials such as brochures, newsletters, news reports, stories, billboards, websites, or used for training. Any photograph, recording, drawing or other illustrative graphic material, audio-visual tape or audio-visual illustration, news report, story or article may be used without my prior examination of the finished product.
By checking the “YES” box, I grant to SC Career Kids, South Carolina ETV, and/or South Carolina First Steps the right to any use of such media. I also agree that SC Career Kids, South Carolina ETV, and/or South Carolina First Steps shall not be required to include any credit identifying any person(s) in the use of the photographs and that such use will be without additional payment, royalties, special credit, or other compensation.
HOLD HARMLESS AGREEMENT
I am the parent/legal guardian of the minor child/children on this registration do hereby agree to assume full responsibility for my child and do agree to indemnify, save, hold harmless, and defend SC Career Kids, Inc., South Carolina ETV and/or South Carolina First Steps and all its employees and agents from any and all liability, claims, demands, actions, judgments, expenses, costs, interest, debts, and fees arising out of, claimed on account of, or in any manner predicated upon any injury, or damage to the property or any person whatsoever, including my child, which may occur resulting from the participation of my child in the SC Career Kids Safety Town.
